Physical Therapy Evaluation

Service Description

This evaluation is performed by a board-certified pediatric physical therapist and is performed at the location of your choice (typically the home). After the assessment the results are discussed with parents and recommendations made with the family's input. We are in-network with Blue Cross Blue Shield PPO and Humana health insurance plans and out of network with other plans. If your plan is out of network, you will receive an invoice of payment that you can submit to your insurance carrier or you can utilize your health spending account.

Cancellation Policy

We understand that having children can sometimes lead to unexpected challenges and changes in schedules. Contact us 24 hrs in advance via email, text, or phone to reschedule your appointment. If you register for a class series and later find out that you are unable to attend please let us know 4-5 days in advance so we can offer your spot to another family.
